Therapeutic Endoscopy

Unlike diagnostic endoscopy, which is primarily used for examination, therapeutic endoscopy involves active treatment to address specific GI problems. It plays a crucial role in the management of bleeding, strictures, polyps, tumors, and bile duct disorders without requiring major surgery.

One of the most common procedures is endoscopic variceal ligation (EVL), also known as banding, which is used to treat esophageal varices (enlarged veins in the esophagus) caused by liver cirrhosis. This procedure prevents life-threatening gastrointestinal bleeding by placing small rubber bands around the varices to reduce blood flow. Similarly, sclerotherapy is performed to inject a special solution into bleeding varices, promoting clot formation and preventing further bleeding.

For patients with polyps in the colon or stomach, endoscopic polypectomy is performed. This procedure involves the removal of abnormal tissue growths (polyps) using a snare or electrocautery, significantly reducing the risk of colorectal cancer and other complications.

Endoscopic retrograde cholangiopancreatography (ERCP) is another vital therapeutic endoscopic technique. ERCP is used to diagnose and treat disorders of the bile ducts and pancreas, such as gallstones, bile duct blockages, strictures, and pancreatic duct disorders. During ERCP, doctors can perform CBD (common bile duct) stone removal, CBD stenting, and pancreatic duct stenting, helping restore normal digestive function and relieve symptoms.

Patients who have difficulty swallowing due to esophageal strictures (narrowing of the esophagus) can benefit from esophageal dilation, where an endoscope is used to gently stretch the narrowed area, allowing for easier food passage. Similarly, PEG tube placement (percutaneous endoscopic gastrostomy) is performed for patients who cannot swallow food normally, providing a direct feeding route into the stomach.

For those suffering from severe acid reflux or stomach ulcers, endoscopic treatment of bleeding ulcers using clips or thermal coagulation ensures rapid bleeding control and ulcer healing. Nasojejunal (NJ) tube placement is also done endoscopically for patients requiring nutritional support due to severe digestive disorders.
